Teacher Participation FORM

The Teacher Participation Form for teachers who have attended Professional Development workshops led by SCFLMTs and are eligible to receive a stipend of $100.00 upon submitting the following evaluation, reflection, and evidence AFTER the session. 

This form does require a Google sign in (personal or professional) due to requested evidence to be uploaded. If you are experiencing issues, please email Chandler.Jordan@moore.sc.edu.
